A further ship was hailed by the IRGC about 11 nm north of Oman. It was told to turn back or be fired on, and it turned back. Lesson: a free-fall lifeboat fell off its ship on passage

DGMA Circular 60 of 2026: on a passage from Kochi to New Mangalore, a free-fall lifeboat was found missing from its cradle. It was recovered three hours later; nobody was hurt. Age and corrosion had thinned the release hook, so it slipped off the holding cam under vibration even though the cam stayed locked. The hydraulic bypass valve was also found open.

What to do: Inspect the release hook and cam every month for corrosion, deformation and any gap. Put the hook's service life in the PMS. Use extra sea lashings only with an approved quick release, and secure the boat as the manual says whenever anyone is inside.
Affects: Ships with free-fall lifeboats; deck and engine officers who maintain LSA When: 22 Sep 2026
